Job Title: Direct Care Worker (DCW)

Required Availability: Flexible Schedule

Age Requirement: 18 or older

Serving various counties in Pennsylvania, Grace Home Healthcare has been an outstanding service provider for elderly people with disabilities and special needs. Funded by The Pennsylvania Department of Health and it’s entities, Grace Home Healthcare has been one of the renown healthcare providers by hiring skilled caregivers that can perform IADLs/ADLs for the needing population. Our Direct Care Worker go through substantial trainings that help provide personalized care and support to clients with disabilities, illnesses, or injuries in a residential or community-based setting. Our caregivers assist the consumers with activities of daily living, promote independence, and maintain a safe and healthy environment.

Major Responsibilities:

1. Personal Care : Assist clients with bathing, dressing, grooming, toileting and other personal hygiene tasks

2. Medication Management : Assist clients with medication administration, monitoring, reminders and documentation

3. Daily Living Activities : Help Client with meal preparation, feeding, cleaning, managing household chores, laundry and errands as needed

4. Mobility and Transfer: Assist clients with mobility, transfers, positioning and re-positioning to prevent injuries and to promote comfort

5. Communication and Socialization : Encourage clients to participate in social activities, communicate with family, friends, relatives and provide emotional support, supervision, coaching/cueing

6. Safety and Emergency Response : Respond to emergencies, such as falls or medical crisis. Provide emotional support until medical help arrives.

7. Documentation and Reporting : Caregivers are an integral part of keeping documentation of consumer medication, records of client care, progress and concerns. Caregivers are also required to report any changes or incidents to the agency as soon as possible.

Requirements:

1. Education :

ü High School diploma is preferred but not required

ü Certification or training in Direct care is preferred but not required

2. Experience:

ü 1-2 years of experience as a direct care worker is preferred but not required

3. Certifications:

ü CPR/First certification in specialized care areas is preferred but required

4. Skills:

ü Compassion, empathy and patience

ü Effective communication and interpersonal skills

ü Ability to work independently

ü Basic math and reading skills

ü Ability to lift, transfer and position the consumer as needed

ü Physical abilities:

ü May require prolonged standing, walking, twisting, sitting, squatting as needed

Working Conditions:

1. Work Environment:

ü Residential or community-based settings, including private homes, group homes, or assisted living facilities

2. Schedule:

ü Based on the clients’ needs, the schedule may vary. This may include days, evenings, overnights, weekends, and holidays.
